Below is the survey sent snail mail. Feel free to copy and paste to the email provided below and send the completed survey.
Now that you have voted for union representation, the first order of business is to hear from all of you as we prepare to negotiate your first contract with Prospect and Centurion. We are sending this survey to every eligible voter. The starting point in negotiations is to make sure that you have all of the terms and conditions of employment at least equal to current union members. For example, union members pay less for medical insurance.
We know from talking to all of you over the last several months that fair and competitive pay rates, pay scales, and annual cost of living and step increases are a priority. And we know that with Prospect for sale, job security is an important issue.
Please rate the following issues by importance 1 being least important and 10 being most important:
___Competitive wage rates.
___Cost of living and step increases every year.
___Job security especially with a new owner.
___Safe and adequate staffing.
___Fairness.
___Save the St. Joseph pension.
___ Rights on the job.
___ Better and less expensive medical plan.
Other issues important to you:

All UNAP locals negotiate collective bargaining agreements that define the terms and conditions of employment for UNAP members. In order for the UNAP to negotiate strong contracts that reflect the goals of the membership, active participation and support from all union members is extremely important.
